Youth Forms and Documents 2011-2012

Electronically Signed at Registration

The following forms are required for all Youth players and are digitally signed during player registration. If you registered your player online these forms have been submitted to Highland Park Boys Lacrosse. A PDF copy is provided here for your records and are not required to be signed and returned to HP Lacrosse.

- Extracurricular and Co-curricular Pledge
- Scot Athlete Agreement

Forms to Print and Return

5/6: HPISD UIL Physical Evaluation and Medical History Form and Participant Emergency Medical Card
7/8: Signed Physical, Participant Emergency Medical Card and signed HP Lacrosse Code of Conduct

All Players in Grades 5-12 must complete and turn in the Participant Medical Emergency Card and (signed by a physician) 2-page HPISD UIL Physical Evaluation and Medical History Form. If your son had a physical at the school in May (only applies to grades 7-12) then please notate this when sending in the Participant Medical Emergency Card. You may document any medical information you want coaches to be aware of on this form. Please note: we don't use any forms from last fall or spring (2010-2011) and new forms are due each school year just like HPISD. If you have not turned in your completed forms and are bringing to practice today, please arrive early.

All players in grades 7-12 must also turn in the Revised HP Lacrosse Code of Conduct. A parent and player signature is required.
